What is Polypharmacy?

Polypharmacy simply means lots of medicines.

Why is taking lots of different medicines a problem?
When you take many different medicines there can be an increased risk of harm from interactions between the drugs. These risks could outweigh the benefits so it’s important to regularly review what you are taking, and why.

What are the benefits of a medicines review?

A medicines review is a meeting to talk about all of your medicines with an expert such as a pharmacist, doctor or nurse. As part of the iSIMPATHY project, medicines reviews are carried out by one of our specialist pharmacists. These pharmacists will work with your wider health care team to help you to get the most from your medicines.

Why do I need a medicines review?
The aim of a medicines review is to ensure that your medicines are helping you with What Matters to You in your life.

When you are first prescribed a medicine it is usually the best one for you, however, things change:

  • You might have developed a side effect
  • Your health may have changed, such as developing a long term condition or a change in a long term condition you already have

Any of these reasons, as well as others can mean the medicine might not be right for you anymore.

During your review, our pharmacists will use the 7-step approach illustrated in the diagram below. Using your personal medication records, the pharmacist will make sure your medicines are right for you at this time. Together, you and the pharmacist will agree a plan which best suits your needs and health now. View our 7-step approach here.

How can I request a medicines review?

At this time, the iSIMPATHY project is only running in a limited number of areas although it is important for everyone to manage their medication as best as they can. See the map and list below to see if reviews are available in your area and who to contact to arrange one.

The Polypharmacy: Manage Medicines app

iSIMPATHY have developed a free app which supports you through your medicine review. It has an easy to use patient and carer toolkit, with information on why you should have a review, what to expect and questions to think about to help you get ready for your review.

The app will help you:

  • Tell your GP or pharmacist about the impact your medicines have on your day to day life
  • Access trustworthy information about your medicines, including benefits, and how to deal with side-effects
  • Keep a personal list of your own medicines
